Registration Forms:
Our Academic and Summer School Application Forms are available in MS Word format. If you would like to begin filling out an enrollment application form you may download and print the form(s) that you need. Please note that each form is at least two pages in length and all pages must be present before they can be accepted by Baker Academy.
Upon clicking the form that you need, a window will open asking you to open the file from its location or save to your hard drive. Choose "Save to Hard Drive", then select the directory you want to save it to. Once the download is complete, you may click on the file to open it.
